Two 19-year-old students and a 65-year-old school caretaker were stabbed to death in Nottingham on 13 June 2023. Three further people were injured in the same series of attacks and survived. In 2024 the attacker, a man with paranoid schizophrenia, was sentenced to an indefinite hospital order after admitting three counts of manslaughter on the grounds of diminished responsibility and three counts of attempted murder. A statutory public inquiry is examining the events, acts and omissions that led to the killings, and has taken evidence from local authorities, health professionals, police officers and the attacker's family. At closing submissions on 8 September 2026, counsel to the inquiry reported the conclusions of a panel of medical professionals that had assessed the emergency response and the survivability of the victims' injuries: more rapid administration of blood products could have improved one victim's chance of reaching hospital and possibly surgery, so that his death was classed as "potentially preventable", although his chance of surviving his injuries was far less than 50 per cent and would have required multiple other rapid and meticulous steps. The other two deaths were found not to have been preventable. The inquiry also addressed evidence that emerged after its hearings had concluded in June 2026. Police had found a holdall at the attacker's former address in the days after the attack containing notebooks that were not disclosed to the inquiry until June 2026, in which he described himself as God and claimed police had mind-reading software. Other documents recovered from a bin at the property contained notes on explosives; the inquiry heard these were not identified or taken into account by police, nor made available to prosecutors or to psychiatric experts. The inquiry was also not told until after the hearings closed of violent assaults he had committed on colleagues in 2021 and in February 2023. Earlier evidence had detailed a string of violent incidents in the years before the attacks that led to him being sectioned four times in two years, and an arrest warrant was outstanding at the time of the attacks after he failed to appear in court over an alleged assault on a police officer. The inquiry's chair called the late disclosure disappointing and directed core participants to provide corrections and omissions by 31 December 2026. A report is due to be published in 2027.
